Yes. You or your provider can request chromosomal microarray (CMA) and whole exome sequencing (WES) raw data. Please submit a Bionano Laboratories “Authorization to Use and Disclose Protected Health Information” release form.
Please submit the form via fax (801-931-6201) or secure upload. We do not accept forms sent via email.
You will receive an email with instructions on how to download the raw data.
Bionano Laboratories can provide guidance and instructions for requesting and receiving your raw data. You are solely responsible for interpretation of the raw data. Bionano Laboratories has previously performed clinical interpretation and reporting of raw data consistent with our reporting standards. Variants not included in the clinical report should be considered research results and should not be used for medical management without appropriate confirmation and interpretation by a qualified genetics professional.
