Bionano Laboratories will contact your insurance provider to determine coverage (both in- and out-of-network) for our clinical diagnostic genetic testing services. This is called a benefit investigation.
Bionano Laboratories will discuss cost with you before beginning if it’s expected to exceed $100 out of pocket. A member of our team will contact you as soon as possible to inform you of your out-of-pocket cost and explain your payment options under our various financial assistance plans.
We understand that the diagnostic journey can be financially challenging. Bionano Laboratories offers prompt-pay discounts, interest-free payment plans, and reductions in out-of-pocket expenses for those who qualify. Eligibility for our financial assistance program is based on need and requires information on family size and income.
Bionano Laboratories may file an appeal with your insurance provider You are only responsible for the cost of the testing that was discussed prior to the test being completed.
Our team will process insurance prior authorizations (PA) on behalf of the family whenever allowed. In addition, we can submit letters of medical necessity (LOMN) and provide supporting clinical documents on behalf of the ordering provider, if necessary
Bionano Laboratories works with all commercial health insurance plans. Coverage of testing is dependent on multiple factors, including but not limited to clinical symptoms, family history and plan specific details.
Bionano Laboratories is currently contracted with some, but not all, state Medicaid programs as well as Tricare. However, Medicare is not currently accepted.
You are responsible for up to the full amount of the test, depending on your deductible, copay, or coinsurance.
Bionano Laboratories provides financial assistance and payment plans in addition to other options. You may also choose to cancel testing.
If you choose to participate in our self-pay option, you are expected to make a partial payment before testing is started. When Bionano Laboratories bills a patient’s insurance for our services, we usually finish the testing and report our results before the claims process is completed. You are sent a statement from Bionano Laboratories that includes your final determination of cost once the claims and appeals process has been exhausted. In certain situations, your cost can be less than the original amount of the unmet deductible, copay, and coinsurance communicated to you.
Bank transfers, credit cards, Pay Pal, or checks are all accepted forms of payment.
It is your responsibility to forward the entire payment to Bionano Laboratories within 10 days of receipt.
